Home Based Primary Care can reduce hospital readmissions

If a patient is planning to leave an acute care setting and transfer home, the Home Based Primary Care model is a perfect solution for this “post acute” or “transitional” care period. Efficient access to a primary care clinician, medication reconciliation and proper coordination of covered home care services by a properly engaged prescriber is the most effective way to reduce hospital readmissions.

Patients that recuperate in a familiar environment, like their home, have much higher success rates.

Patient Qualifications:

  1. Enrolled in either Medicare Part B or one of our contracted Medicare Advantage Plans
  2. Lives in our Service Area – Manhattan, Bronx, Queens, Brooklyn, Westchester, Nassau
  3. Has physical or cognitive functional limitations
  4. Suffers from chronic illness
  5. Cannot otherwise access proper Primary Care services
